MARRIAGE LICENSE APPLICATION

Marriage License issuance is limited to Brookhaven Town Residents only at this time.

Applications submitted without all required documentation will be rejected.

Please use the form below to submit a marriage license application from the Town Clerk's Office. Please note, an in-person appointment will be required to complete the marriage license application process.

In addition to submission of this form, each party to the marriage must present the following:

  1. A Birth Document
    1. Original or Certified Copy of Birth Certificate
    2. Baptismal Certificate with birth name, date of birth, place of birth, parents’ names and seal of the church
  2. Valid Government Issued Photo Identification
    1. Driver License
    2. DMV Non-driver ID
    3. Passport
    4. US Military ID
    5. Permanent Resident Card
  3. Proof of Dissolution of ALL Prior Marriages, if any
    1. If divorced, applicant(s) must bring the Divorce Judgments from ALL previous marriages
    2. If the prior marriage ended by death of a spouse, a certified death certificate must be presented listing the applicant as the surviving spouse
  4. Payment of Marriage License Fee in the Amount of $40.00
  5. Both Parties must Appear at a Scheduled Appointment Time to Execute the Marriage License (this language may change depending on if we can utilize an online appointment system)

If the above documentation is not in English, the document must be translated pursuant to the specifications as set forth here.

Please allow 5 business days for review of your online marriage license application. Please note, appointments are currently booking approximately four (4) weeks out from the approval of applications.

Original or Certified Copy of Birth Certificate OR
Baptismal Certificate with birth name, date of birth, place of birth, parents’ names and seal of the church
Original or Certified Copy of Birth Document must also be presented at time of appointment.



Driver License OR
DMV Non-driver ID OR
Passport OR
US Military ID OR
Permanent Resident Card OR
Valid Government Issued Photo Identification must also be presented at time of appointment.



If divorced, applicant(s) must upload the Divorce Judgments from ALL previous marriages OR

If the prior marriage ended by death of a spouse, a certified death certificate must be presented listing the applicant as the surviving spouse

Proof of Dissolution of all prior marriages must also be presented at time of appointment.




If divorced, applicant(s) must upload the Divorce Judgments from ALL previous marriages OR

If the prior marriage ended by death of a spouse, a certified death certificate must be presented listing the applicant as the surviving spouse

Proof of Dissolution of all prior marriages must also be presented at time of appointment.




If divorced, applicant(s) must upload the Divorce Judgments from ALL previous marriages OR

If the prior marriage ended by death of a spouse, a certified death certificate must be presented listing the applicant as the surviving spouse

Proof of Dissolution of all prior marriages must also be presented at time of appointment.




Translation from bona fide business, university, embassy or consulate

Translation on company, university or government letterhead with original document attached

Translation completed from the original document, not a scan, fax or photocopy

Translator must attest to accuracy of translation and competency to translate the specific language

Translation must include translator’s original notarized signature
